GABR Children’s
Description


Applications will be considered throughout the year. Funds will be awarded on a first come, first served basis.


Applicants must meet the following criteria:

  • Be under 18 years old at the time of submission.
  • Reside within the service area.
  • Meet financial eligibility requirements.
  • Be a patient at Sanford Health in the Bismarck region.
  • Have a diagnosis defined as a chronic condition, behavioral health condition, or an acute condition with an inpatient hospital stay.
  • Exhaust all other funding assistance options.
  • Adhere to a maximum of $1,500 per calendar year.

Chronic conditions may include, but are not limited to cancer, severe diabetes, cystic fibrosis, cerebral palsy or similar conditions, severe autism.


Funding will be provided for the following expenses within 6 months from the time of submission and not provided prior to the service: 

  • Traditional medical treatment. This includes things like: hospitalization, therapy, clinic visits for chronic conditions.
  • Travel to medical treatment. Assistance for medical travel will be reimbursed at $100 per night for lodging, $50 per day for meals and $0.22 per mile.
  • Specialized medical equipment
  • Prescriptions


You will be asked to provide the following supporting documentation:

  • Most recent tax return showing number in household and total taxable income.
  • Letter of support or a prescription from your healthcare provider.
  • Insurance denial or proof of non-covered item.
  • Itemized receipts or invoice.
