The Sanford Health Foundation supports patients at Sanford Children's Hospital in Sioux Falls, a proud member of Children’s Miracle Network Hospitals, by providing specialized equipment, important unbilled family services and direct child and family assistance for expenses not covered by insurance. There is a maximum allowance of $500 per child per fiscal year. Please complete the full application, including how the assistance will be used, travel dates, appointment dates, travel destination and purpose.
For travel assistance, the Sanford Health Foundation is able to assist with $40 a night for lodging, $20 a day for meals with an overnight stay and $0.25 a mile for mileage. If you would like the assistance to be provided prior to a trip, please allow two weeks for processing. Otherwise assistance can be provided any time following the trip within the same fiscal year.
In addition, the Sanford Health Foundation is able to assist with diabetic supplies, medication expenses and special food and formula for children. Other expenses related to a child’s medical care will be considered on a case-by-case basis. Receipts and verification of expenses for these items must be submitted.
